If you require a scan for one of your patients, please complete the request form found in the “request scan” tab of this website and email securely to adrian@ardenforestultrasound.secure-comm.com

Once the request form has been sent, ask your patient to contact Arden Forest Ultrasound directly to register, where they will be provided with a welcome pack (this will be tailored to the information you send in on the request form)

You will be sent an email to set up a secure portal for communication thereafter, where you will be asked to create your own PIN which you can use to receive the communications.
